Categories of records held
- Patient records: clinical notes, referral letters, results, imaging and correspondence
- Consent records, including offshore processing, guardian and AI consent
- Billing, medical aid and pre-authorisation records
- Employment and human resources records
- Company, statutory, tax and financial records
- Agreements with operators and suppliers
Patients may also ask for their own health records directly, in terms of the National Health Act and POPIA, without following the full PAIA procedure.
How to request records
- Complete the prescribed request form (Form 2), available from the Information Regulator.
- Send it to the Information Officer, with proof of identity.
- Prescribed fees may apply, as set out in the PAIA regulations. You are told of any fee before the request is processed.
- The practice responds within 30 days, unless an extension is permitted by the Act.
